Position Title: Client Care Navigator (Bilingual)

Location: Beresford, NB

Reports to: Provincial Manager, Service and Operations

Term: Full Time, Permanent

Date: 28th July, 2023

Scope and Summary of Primary Responsibilities

The Client Care Navigator (CCN) will be the key contact for individuals after a vision loss diagnosis. The responsibilities include assessment, supporting clients to achieve their care plan goals, and strengthening the communication linkages between VLRC staff and health care providers.

What is a Day in the Life of a Client Care Navigator?

  • Schedules all VLRC therapy appointments with primary service provider and specialized service provider.
  • Seeks feedback on client satisfaction and takes remedial action as required.
  • Monitors and provides proactive follow-up for clients to ensure ongoing collaboration across services/providers.
  • Ensures that all service-related activities provided are documented in the client health record following VLRC policies.
  • Interacts with clients, their family, and caregivers to develop and identify vision related goals.
  • Educates community partners about Vision Loss Rehabilitation Canada and its specialized services offered to people with vision loss.
  • Requirements

    What You Need To Be Successful in this position:

    • Bachelor's degree in Social Work, Education, Gerontology, Nursing, or related healthcare field.
    • Ability to communicate in French and English is essential.
    • Knowledge of client-centered philosophy in healthcare including care planning, assessments and coordination.
    • Time management and prioritization skills, with the ability to be resourceful and take initiative.
    • Computer skills, organized with an attention for detail.
