Position Title: Client Care Navigator (Bilingual)
Location: Beresford, NB
Reports to: Provincial Manager, Service and Operations
Term: Full Time, Permanent

Scope and Summary of Primary Responsibilities
The Client Care Navigator (CCN) will be the key contact for individuals after a vision loss diagnosis. The responsibilities include assessment, supporting clients to achieve their care plan goals, and strengthening the communication linkages between VLRC staff and health care providers.

What You Need To Be Successful in this position:
- Bachelor's degree in Social Work, Education, Gerontology, Nursing, or related healthcare field.
- Ability to communicate in French and English is essential.
- Knowledge of client-centered philosophy in healthcare including care planning, assessments and coordination.
- Time management and prioritization skills, with the ability to be resourceful and take initiative.
- Computer skills, organized with an attention for detail.
