Position Title: ProjectCoordinator, Diabetic Retinopathy Screening Program

Reports To: Regional Manager

Date: March 17, 2021

Duration: 6 month contract

Salary: $24.73 $27.04/$25.85 $32.04

Requirements

I. Scope and Summary of Primary Responsibilities

Reporting to the Regional Manager the Project Coordinator is responsible for the development and implementation of the DR Screening project in Ontario.

The ProjectCoordinator ensures partners and stakeholders have access to project deliverables to integrate best practices into the continuum of healthcare services for people with diabetic retinopathy. They achieve this mission by developing and maintaining strong working relationships with stakeholders including patient groups, local, community health care agencies and other community partners. Ensures compliance to operational best practices and in accordance with VLRC policies, standards, systems and procedures; monitors and measures departmental performance in relation to internal and external accountability targets.

The accountability includes building and nurturing relationships, communication and reporting. As well as leveraging knowledge, skills, and opportunities to generate a strong community of stakeholders and partners.

Achievement of strategic objectives for the project will be measured against goals set for the organization.

III. Essential Functions

The position entails, but is not limited to the following:

  • Develop and maintain good relations and ongoing communication with other agencies, consumer groups, and community professionals.
  • Ensure effective coordination, integration and referrals into and out of the specialized programs.
  • Responsible for the coordination of professional development and training of staff as appropriate
  • Monitor Key Performance Indicators including participant satisfaction survey, outcome evaluation and efficiency results
  • Identify the educational and training needs of partners and develop program and training seminars as needed.
  • Track progress toward varied regional program goals
  • Conduct ongoing project reviews and develop reports for internal and external use
Relationships
  • Liaises and builds relationships with various internal and external stakeholders to support and implement program goals.
  • Manages multiple relationships through diplomacy, information, value-added expertise and facilitation.

  • Ability to provide solutions to routine as well as complicated problems utilising creativity, and problem solving skills.
  • Respond to and resolve complaints, in accordance with the complaint policy.

Knowledge and Skill Requirements:
